Accident / Incident Report

This form is required and must be completed following any incident. Once you have all necessary information, it should take approximately 10 minutes to complete.

Ensure safety first
Check that everyone is safe and move to a secure location if possible.
Call 911
Call 911 immediately if there are any injuries or another vehicle involved.
Contact the police
- If another vehicle is involved, you must obtain a police report.
- Request a case number or a copy of the report.
- For non-emergencies, you may contact:

Document the scene
- Take clear photos of all vehicles (front, sides, and rear), including visible license plates.
- Capture the surrounding area and any relevant road conditions.
- Photograph the other driver’s license and insurance card.
Minor damage (no other vehicles involved)
If there is only minor damage and no other vehicles are involved, please proceed with documentation and reporting.
